Job Overview We are currently seeking a dedicated and skilled Band 6 Occupational Therapist for a community-based role at Leeds Community Healthcare. The successful candidate will work across various community settings, providing patient-centered care and promoting independence. Key Responsibilities Assess, plan, and deliver occupational therapy interventions for patients in the community. Work with multi-disciplinary teams to support patient rehabilitation and recovery. Develop and implement tailored care plans to meet individual needs. Provide advice and education to patients, families, and carers. Manage a caseload, ensuring timely and effective interventions. Key Requirements HCPC registration as an Occupational Therapist. Proven experience in community-based occupational therapy. Strong communication and interpersonal skills. Ability to work independently and as part of a team. Experience in managing complex caseloads is desirable.
