Join Our Hospital Discharge Team Social Care Assessor Opportunity Are you ready to make a meaningful difference in the lives of patients as they transition from hospital to home? We re seeking a Social Care Assessor to join our Hospital Discharge Team on an initial 3-month contract , with the possibility of extension . This part-time position is for 17.5 hours per week . What You ll Do: Assess and support patients social care needs as they prepare to leave the hospital. Work closely with healthcare professionals to ensure safe and effective discharge plans. Ensure compliance with national and departmental guidelines while delivering high-quality care. Provide vital support to individuals during a critical time in their recovery journey. What We re Looking For: Experience in social care assessments, ideally in a hospital or health-related environment. A compassionate and empathetic approach to supporting vulnerable individuals. Strong communication skills and the ability to collaborate effectively with multi-disciplinary teams. Someone who is proactive, adaptable, and dedicated to improving patient outcomes. This is an initial 3-month contract with the possibility of extension .
