We are seeking a dedicated and proactive Clinical Pharmacist to join a team in Swindon. This is a fantastic opportunity to play a key role in delivering high-quality patient care within a dynamic and supportive healthcare environment.As a Clinical Pharmacist, you will work closely with GPs, nurses, and other healthcare professionals to provide expert medication management, improve patient outcomes, and support prescribing practices. \n Key Responsibilities:\n \n Provide expert clinical advice on the safe and effective use of medicines. \n Conduct structured medication reviews to optimise patient treatment plans. \n Support prescribing decisions to ensure cost-effective and evidence-based practices. \n Offer guidance on long-term condition management, including diabetes, hypertension, and respiratory diseases. \n \n About You:\n \n A degree in Pharmacy (MPharm or equivalent). \n Registration with the General Pharmaceutical Council (GPhC) . \n Experience in a primary care, hospital, or community pharmacy setting. \n A postgraduate qualification in clinical pharmacy (desirable but not essential). \n \n For more information please get in touch on 07907860572 and Ibraim would be happy to discuss this opportunity further. \n Tagged as: Clinical Pharmacist, Pharmacy, Swindon
