Note: The total care for a patient caseload is assigned to a staff group (a team). The team leader is responsible for the provision of care of these patients through the following job functions:
Receives referrals and makes initial patient evaluation.
Coordinate admissions.
Initiates and coordinates the patient's plan of treatment through the contacting of community services, therapists, and/or social workers, and through staff assignment to provide patient care.
Directs patient care.
Educates/supervises patient family.
Reviews and provides supervision for each patient's plan of care.
Assures completion of 60 day record review of each patient (recertifications).
Coordinates transfer and discharge of patients.
Conducts periodic chart audits.
Staffing/scheduling.
Assures financial and regulatory compliance affecting patient care team.
Checks aide notes for accuracy, and completeness prior to signing.
Assists with employee education, disciplinary procedures, evaluation, and termination.
Participate in QA/QI process.
Assists in formulation, revision, implementation, and evaluation of policies and procedures.
Is responsible for adhering to all practice standards as they apply to patient care.
Job Requirements:
Current RN licensure in state of employment.
One (1) year experience in home health preferred.
Current driver's license and vehicle insurance in state of employment, access to a dependable vehicle, and the ability to travel within a 50 mile radius on a daily basis.
